How can I become a patient?
You must have a referral from a physician that is currently treating your pain. Their office can ask for one of our fax referral forms, and fax it along with the other information needed to our office. After we receive the form, the nurse will review the notes, and we will call you to schedule your appointment with Dr. Bojrab as soon as possible.
How can I get my prescription refilled?

The Name and Strength of the Medication(s) to be Refilled
The Name and Location of the Pharmacy
If the Pharmacy is Not Stated, Medications will be Sent to the Previous Pharmacy on File

How can I find out my X-ray/ Test results?
Make a follow-up appointment to review the results with your doctor. You will not be able to obtain your results over the phone. Dr. Bojrab will review all x-ray/ test results. However, it is not feasible for our office staff to contact each patient regarding their results. We ask that you do not call our office and ask us to review your results.
How do I schedule an appointment?
Pain Management requires that all patients be referred by their personal physician. Talk to your doctor about being referred to us. We cannot accept walk-ins. Our business hours are 8 a.m.- 5 p.m., Monday through Friday.

What happens if I can’t make an appointment?
If you cannot keep your appointment, please call our office 24 hours in advance to cancel. If you do not contact us, then a $25.00 fee will be assessed to your account. For scheduled block visits, a $100.00 fee will be assessed if no 24 hours notice is not received.
